If you or anyone in your household have experienced any of the following possible symptoms of COVID-19 within the last 14 days.

Fever
Fatigue
Dry cough
Difficulty breathing
Severe pain or pressure in the chest
New loss of taste or smell

If you or anyone in your household has been diagnosed with or have knowingly been exposed to anyone diagnosed with COVID-19 within the last 30 days.

If you or anyone in your household has traveled outside the US, or to any locality that is, or has been, a “hot spot” for COVID-19 cases within the last 30 days.
